Please tell the receptionist the purpose
of your visit so an appropriate amount
of time can be scheduled for your appointment.

On your first visit, please bring your insurance, Medicare or Medicaid cards,
as well as referral forms where needed.

Plan to arrive about 10 minutes prior to your appointment to fill out our history forms.

To Cancel an Appointment - If you must cancel an appointment, kindly give 24 hours notice.

Emergencies

If an emergency occurs, call our regular office number. Telephone calls are received 24 hours daily, and emergency information can be dispatched to the doctor for prompt care. If the problem is something that cannot wait, proceed to the nearest emergency room and the doctor there can contact Dr. Lunt.
