For this reason, it is essential that you provide us with your insurance phone number and policy numbers. Payment is due at the time of service. In those instances where we accept assignment from your insurance, your co payment needs to be paid at the time of service. While filling out the insurance claim form is a courtesy we extend to our patients, all charges are your responsibility.

The fee for treatment is based on the complexity of the condition.
